We would like to thank you for visiting the Aliso Viejo Family Dental Group . Our primary goal is to make your first visit to be comfortable and informative of your personal dental needs.
Because the foundation of a long term relationship is established at your first visit , with your cooperation, and use of the most recent technology we will complete a thorough and comprehensive dental evaluation. to properly diagnose your needs,. and create a treatment plan that would remedy all the issues and yet be affordable financially.
X-Rays
If your dentist has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them with you. If additional x-rays are necessary, they can be taken at our office.
IMPORTANT:
All patients under the age of 18 must be accompanied by a parent or guardian.
Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, Blood thinner, anticoagulant therapy, etc.).
